Scheduling an Appointment

We only see patients with a referral from their eye-care doctor. It is essential for your doctor to send us a letter about your condition. We will contact him/her promptly if we have any questions. This process will enable us to determine whether you should make an appointment in our clinic.

We also ask that the Informed Consent Form also be filled out in full and signed prior to being accepted as a patient
