Scheduling an Appointment

In order to determine whether you should make an appointment in our clinic, we will need recent, detailed information from your eye doctor.  Typically, it is most helpful to receive this as a narrative letter of referral.  Short of this, a copy of your last comprehensive eye exam will be a good place to start.
All information should be forwarded to our Director of New Patient Affairs either via fax, email or post.
